Donations to The Five Star Heart Project can be made online through PayPal (donate button below) OR by mailing a check directly to The Five Star Heart Project at the address below along with a completed sponsorship package. Should you elect to donate via PayPal, please complete and mail the sponsorship form OR complete and submit the electronic form below. All businesses shall receive a Letter of Acknowledgement from Five Star Heart in a timely manner after receipt of sponsorship funds or in-kind donations.
